The following risk assessment form is to be completed by each care manager/carer prior to clients admission to Active Lives Ltd.

It is also referred to and updated at each review if necessary.

The risk assessment should only be used as a guidance and is based on the information available at the time.

Although not a legal document, it is a way of recording relevant information when a client/student presents sufficient risk to themselves or others and details should be documented as part of good practice.
